Online Recertification Submission

Recertification RECERTIFICATION - Initial certification is valid for five (5) years. The CBSPD’s Board identified that the usual period for significant change in technology/knowledge for our profession is five years (similar to AORN, APIC), and therefore the Board determined that five years is an appropriate cycle.

This page is for online recertification submission only.

All applicable form fields must be completed.

The cost for recertification is $135.00 (only one recertification type per submission is allowed).

You can ONLY use this online submission to submit your on-going CEU record that has been signed by your Supervisor/Manager on facility letterhead.

The online submission form CANNOT be used to submit numerous certificates. You will need to download the recertification packet to send your application and supporting documents by mail.

The standard processing fee is $135.00. Expedited review of your re-certification application will cost an additional $50.00. This service is only available for the month of certification or extension. No refunds will be allowed for this service.
You must include your employment verification letter(s) to receive credit for full-time, part-time, or per diem positions (this must be within the last five years). Please include this in the upload of your Ongoing CEU Record.
